Registered Mental Health Nurse (RMN) – Enhanced Supported Living Environment
We are seeking a skilled Registered Mental Health Nurse (RMN) to take responsibility for the assessment, planning, implementation, and evaluation of care for individuals with complex and challenging needs in an enhanced supported living environment. The post holder will provide specialist mental health care and work collaboratively with internal and external agencies to promote the well-being and recovery of the people we support. You would be based 3 days a week at our Westhoughton service and 2 days a week out on assessments across the Northwest supporting our assessment practitioners assessing individuals for our services.
Key Responsibilities:
- Comprehensive Mental Health Assessment: Develop transition, admission, and discharge plans, follow-up referrals and assessment for suitability for the services, undertake assessments of individuals including those residents presenting with higher levels of risk, develop the care plan alongside the person we support, review risk factors and amend accordingly and participate in the planning, development and evaluation of professional specific and local multi professional service.
- Care Planning and Risk Management: Develop and implement person centred care plans and risk management plans for people we support, ensuring that these are regularly evaluated and updated.
- Transition, Admission, and Discharge Planning: Facilitate smooth transitions for individuals entering or leaving the service, following up on referrals and conducting assessments to determine suitability for the services.
- Evidence-Based Interventions: Deliver individualized interventions and therapeutic care tailored to the needs of those with complex mental health issues. Use recovery-focused strategies to enhance the well-being of vulnerable adults.
- Collaboration with External Agencies: Liaise with secondary care providers, specialist agencies, and external stakeholders to ensure cohesive treatment planning and continuity of care for individuals.
- On-Call Duties: Participate in the out-of-hours on-call rota to provide timely and effective support when necessary.
- Medication Management: Oversee medication audits and ensure staff competency in handling medications. Monitor adherence to the Mental Health Act (MHA) requirements, such as Community Treatment Orders (CTO).
- Safeguarding and Legal Compliance: Safeguard each individual from any form of abuse (physical, psychological, financial, sexual, institutional, or unintentional) by promptly reporting any concerns. Ensure compliance with the Mental Capacity Act (MCA) and related legislation, following the correct procedures when individuals lack mental capacity.
- Documentation and Confidentiality: Ensure all care documentation is timely, accurate, and adheres to legal, organisational, and ethical requirements, maintaining strict confidentiality.
- Positive Therapeutic Risk Management: Manage high-risk and complex presentations in a community setting, using positive therapeutic risk management approaches.
- Multi-disciplinary Teamwork: Work collaboratively with the multidisciplinary team, including external health and social care professionals, to coordinate care plans and ensure the best outcomes for the people we support.
- Specialist Mental Health Knowledge: Maintain up-to-date knowledge of mental health legislation, including the Mental Capacity Act (MCA), Mental Health Act (MHA), and any relevant policies associated with learning disabilities and mental health care.
Skills and Experience Required:
- Proven ability to offer comprehensive mental health assessments.
- Experience in developing and implementing care and treatment plans for individuals with complex mental health needs.
- In-depth knowledge of relevant mental health legislation, including MCA and MHA.
- Experience managing medication and leading medication audits.
- Strong safeguarding awareness and experience in working with vulnerable adults.
- Ability to manage complex cases in community settings with positive therapeutic risk management.
- Excellent communication and collaboration skills, particularly in working with external agencies.
- It is essential that you have a current driving license and vehicle - this role is to cover all of our Northwest services.
Salary: £36,000 per year
