We currently support over 150 individuals across the Northwest, Cumbria, Yorkshire, and the Midlands — and we’re growing. Now, we’re looking for an enthusiastic and motivated Assessment Practitioner to join our exceptional team.
🧠 What You’ll Do
As an Assessment Practitioner, you’ll take the lead in clinically assessing individuals to determine the right level of care and support within our services. Your clinical insight, compassion, and strategic thinking will directly shape recovery journeys and transitions for potential new residents.
Key Responsibilities:
Lead clinical assessments for individuals with complex needs
Collaborate with commercial and operational teams in the referral process
Develop person-centred transition, admission, and discharge plans
Offer specialist clinical advice and risk assessments
Provide on-site support for new residents and contribute to care planning
Ensure all practices are evidence-based and aligned with professional guidelines
🎓 What We’re Looking For
We’re seeking a passionate and experienced clinician who’s ready to hit the ground running.
Essential Qualifications & Experience:
Registered professional (RMN, OT, or Social Worker) with valid registration
3+ years’ post-qualification experience in mental health or LD services
Skilled in clinical assessment, MDT collaboration, and risk formulation
Familiar with relevant legislation (Mental Health Act, Safeguarding, etc.)
Full UK driving license and flexibility to travel

A confident communicator with strong time management skillsCommitted to high-quality care and safeguarding best practice
